Cole, who contracted the tropical disease while holidaying in Tanzania with Hough last month, complained of feeling feverish and lethargic.
Hough has stayed by the former Girls Aloud star's side during her California break.
"Cheryl is doing her best to put weight on and relax but she is not finding it easy. She has been feeling particularly unwell since she got to LA and everyone is very worried," a close source said.
"She says she has lost her energy. She lost 10lbs while she had malaria and is still much skinnier than would be ideal."
The Three Little Words singer will return to the UK at the end of month to resume work on the X-Factor.
